Due to a major plumbing problem we have taken the difficult decision to close the surgery on the above date. Whilst we will not be able to have any patient f2f appointments we will still be offering telephone consultations.

Patients requiring prescriptions or fitness to work certificates will be required to request these via our website or Practice e-mail address: swlicb.collierswoodsurgery@nhs.net

Regrettably, we will not be able to issue test request forms as there will be no patient access. If you need to collect a test request form please arrange to collect it today (Thursday) or Monday 20th November.

It is hoped that the required work will be able to be completed before Monday 20th November, all updates will be posted on this website.

Our phone lines will be operational as per usual but staff will not be on site.

The Practice appreciates the inconvenience that this decision may cause and would like to thank you for your understanding.
